Summer Performance Student Information Form
Complete the Summer Performance Student Information form. Berklee needs the following information for administration purposes during the program, including a passport number and emergency contact information. Please note that minors under 18 must also provide the passport number for one of their parents/guardians.
Complete your Personal Student Agreement
Soon after completing the form above you will receive a DocuSign email from Student Affairs giving you access to a personal student agreement, which you will need to complete and digitally sign. Please note that it will be the parents/legal guardians of students under 18 years of age who will first receive the DocuSign email containing the student agreement; therefore it is imperative that students provide a valid email address for their parent/legal guardian in the emergency contact Information form.
Complete your Image Rights Form
Complete the image rights form. Berklee requires the following information for administration purposes during the academic year. Complete this form as soon as possible. Please note that if the student is a minor—according to their national law—this form shall be entirely filled out and accepted by parental authority/guardian.
Submit Summer Student ID Headshot
Complete this form and submit a headshot. This is an essential step to access the Valencia campus.
