Soon after completing the form above you will receive a DocuSign email from Student Affairs giving you access to a personal student agreement, which you will need to complete and digitally sign. Most information in the student agreement will have been automatically populated with the information submitted in the Berklee Student Information form. The deadline to complete the personal student agreement is Monday July, 15.

As a Berklee affiliate, we will provide you with an official Berklee ID card. Complete this form as soon as possible and follow these instructions for submitting your photo electronically for your Berklee student's ID. The deadline to complete this form is Monday, July 15.
